Inside the Programme: How Vibrant Health Advocates Cobalt Keeps Clydebank's Oldest Residents on Their Feet

A look at the day-to-day work of a small West Dunbartonshire charity that is quietly preventing the falls, hospitalisations, and care home admissions that older people fear most.

Clydebank is a town that knows what it means to look after its own. The community that rebuilt itself after the Blitz, that survived the collapse of the industries that had defined it for a century, has never been short of collective resilience. But resilience has limits, and for the town's growing population of over-75s, the practical business of ageing safely at home increasingly requires more than community spirit. It requires coordination, expertise, and the kind of patient, unhurried attention that statutory services, stretched thin across the whole of West Dunbartonshire, cannot always provide.

That is the gap that Vibrant Health Advocates Cobalt was established to fill. The charity works exclusively with older residents of Clydebank, focusing on the cohort most at risk of falls and most likely to lose their independence as a result: people aged 75 and over, many of whom have lived in their homes for decades and are determined to remain there.

The core of the charity's work is the home-safety assessment — a structured visit that goes considerably further than a quick look around. Support workers are trained to assess physical fall risk factors including gait, balance, grip strength, and medication interactions, as well as environmental hazards in the home itself. The assessment produces a written plan, and the charity then helps to implement it, coordinating with GPs, pharmacists, occupational therapists, and local handyperson services to make the changes that will actually reduce risk.

What distinguishes the charity's approach is the emphasis on follow-through. That follow-through takes time, but it is also where much of the real impact happens. Research consistently shows that fall-prevention advice delivered without implementation support produces only modest results. The support to actually make changes is what moves the needle.

The charity also runs group balance and strength sessions at accessible venues across Clydebank — community halls and church annexes where older people can work on the physical foundations of fall prevention in a social setting. The sessions are led by trained instructors and designed for people who may not have exercised in years, removing the barriers of cost, transport, and intimidation that keep many older people out of conventional gym or leisure-centre programmes.

The numbers that the charity tracks are grimly clarifying. A single hip fracture in an older person costs the NHS an estimated £14,000 to treat, and that is before accounting for rehabilitation, ongoing care needs, or the loss of independence that frequently follows. Falls are the leading cause of injury death in the over-75 age group in Scotland. The interventions that prevent them are, by comparison, inexpensive — a grab rail, a medication review, a set of non-slip slippers, and someone to make sure those things actually happen.

For the people who benefit from this work, though, the statistics are beside the point. What matters is the flat on Kilbowie Road, the garden they have tended for forty years, the neighbourhood where everyone knows their name. Vibrant Health Advocates Cobalt exists to help them keep it.
